Michigan State University Extension offers the Chronic Disease Self-Management Program, which is called Personal Action Toward Health (PATH) in Michigan. These educational programs are designed to help people living with chronic conditions (and their caregivers) gain confidence, develop practical skills and take an active role in their health.

MSU Extension offers specialized PATH workshops for:

  • Chronic Disease PATH
  • Diabetes PATH
  • Chronic Pain PATH

PATH program participants learn how to:

  • Set realistic and meaningful health goals
  • Manage symptoms and stress more effectively
  • Increase physical activity and energy
  • Communicate confidently with healthcare professionals
  • Stay motivated and connected to support systems
